Disclosure Process

STEP 1 | Outside Activity Prior Approval

  • If you are engaging in or plan to engage in an Outside Activity in which you will spend one (1) day or more per year, complete an Outside Activity Prior Approval form.
  • Repeat for each applicable Outside Activity.
NOTE: If you are not engaging in or planning to engage in an Outside Activity in which you will spend one (1) day or more per year - SKIP TO STEP 2.

STEP 2 | Outside Activity Prior Approval

  • Review, update as needed, and submit your annual Conflict of Interest form.
